Prostate enlargement, medically known as Benign Prostatic Hyperplasia (BPH), is one of the most common health problems in Indian men over the age of 50. Studies suggest that nearly 50% of men above 50 and 80% of men above 70 have some degree of prostate enlargement. Despite being so common, many men suffer in silence due to hesitation or lack of awareness.

Dr. Pragnesh Desai, Senior Director – Urology at Yatharth Super Specialty Hospital, Noida Extension, has treated thousands of prostate patients and offers the most advanced, minimally invasive treatments available in India.

What is the Prostate?

The prostate is a small gland (about the size of a walnut) found only in men, located below the bladder and surrounding the urethra (the tube that carries urine). As men age, the prostate naturally enlarges. When it grows too large, it squeezes the urethra and causes urinary problems.

Symptoms of Prostate Enlargement (BPH)

Watch out for these warning signs:

  • Weak or slow urine stream
  • Difficulty starting urination (straining)
  • Feeling that the bladder is not fully empty
  • Frequent urination — especially at night (nocturia)
  • Sudden urge to urinate (urgency)
  • Dribbling at the end of urination
  • In severe cases — complete inability to urinate (urinary retention) — this is a medical emergency

Difference Between BPH and Prostate Cancer

BPH (enlarged prostate) is NOT cancer and does not increase your risk of prostate cancer. However, both can cause similar urinary symptoms. A PSA blood test and digital rectal examination help distinguish them. Always consult a urologist for proper evaluation.

Treatment Options for Prostate Enlargement

1. Medications (Mild to Moderate BPH)

  • Alpha-blockers (e.g. Tamsulosin) – Relax prostate and bladder muscles for better urine flow
  • 5-Alpha Reductase Inhibitors (e.g. Finasteride) – Shrink the prostate over time
  • Combination therapy for better results

2. HoLEP (Holmium Laser Enucleation of the Prostate) ⭐ Gold Standard

HoLEP is the most advanced and permanent surgical treatment for BPH. A laser is used to remove the enlarged prostate tissue through the urethra — no cuts or incisions on the body. Benefits:

  • Works for all prostate sizes
  • Minimal blood loss
  • Hospital stay: 1–2 days
  • Long-term results — rarely needs repeat surgery
  • Catheter removed within 24 hours

3. ThuFLEP (Thulium Fiber Laser Enucleation)

The newest laser technology for prostate surgery, offering even more precise tissue removal with excellent haemostasis. Dr. Pragnesh Desai is one of the few surgeons in NCR performing ThuFLEP.

4. TURP (Transurethral Resection of Prostate)

Traditional gold standard — electric loop removes prostate tissue. Still widely used, especially for medium-sized prostates.

5. Robotic/Laparoscopic Simple Prostatectomy

For very large prostates (above 100 grams), robotic or laparoscopic surgery is preferred. Dr. Desai performs these routinely at Yatharth Hospital.
